ATSB Investigation: 'Normalised' Flights After Dark Led to Fatal R22 Helicopter Crash
The Australian Transport Safety Bureau has concluded its investigation into a fatal Robinson R22 helicopter accident near Mangalore, Victoria. The report found the pilot, who was not rated for night flight, likely experienced spatial disorientation in dark, moonless conditions. Investigators determined that the operator had 'normalised' the practice of conducting flights after last light, despite the risks. This acceptance of a previously unacceptable practice created an elevated and unmanaged risk, directly contributing to the accident.
