NTSB Preliminary Report: Pilot in Fatal Westerly Crash Was on Post-Maintenance Test Flight
The National Transportation Safety Board has released its preliminary report on the fatal August 27 crash at Westerly State Airport. The pilot, Richard M. Monaghan, was conducting a post-maintenance test flight in his Seamax M-22 amphibious light sport aircraft. Recent maintenance had been performed to replace a broken throttle cable. Witnesses reported seeing the aircraft take off, make a left turn, and then enter a steep descent into a wooded area. The NTSB's investigation is ongoing, and a final report will be released at a later date.
