NTSB Launches Go-Team to Investigate Fatal Mid-Air Collision in Arizona
The National Transportation Safety Board is launching a Go-Team to investigate a fatal mid-air collision near Chandler, Arizona. The incident involved a Cirrus SR22 airplane and a Rotorway 162F helicopter. The collision occurred on October 1, 2021, resulting in a post-crash fire. Federal investigators confirmed that the accident tragically resulted in two fatalities. The NTSB team, led by investigator-in-charge Mike Folkerts, will conduct a thorough on-site investigation to determine the cause.
