NTSB Cites Safety Failures and Lack of Oversight in Deadly Dallas Air Show Collision
The National Transportation Safety Board has released its final report on the fatal 2022 mid-air collision at the Wings Over Dallas air show. The investigation identifies the probable cause as the air boss's inadequate oversight and the absence of an integrated safety plan for all pilots. Investigators found there was no joint pre-flight briefing, and the separate flight groups used different radio frequencies. The report also notes that the P-63 pilot's view was likely obstructed, and a lack of required GPS tracking contributed to poor situational awareness. The NTSB has recommended the FAA mandate comprehensive safety management systems for air shows.
