NTSB Cites Pilot Error, Excessive Speed in Fatal 2019 Amazon Cargo Jet Crash
The National Transportation Safety Board has determined the probable cause of the fatal 2019 Atlas Air crash operating for Amazon. Investigators concluded that the first officer reacted inappropriately to an inadvertent activation of the go-around switch, leading to spatial disorientation. This caused him to put the Boeing 767 into a steep, high-speed dive from which the crew could not recover. The crash, which occurred near Houston, killed the two pilots and a passenger. The NTSB also cited the captain's failure to monitor and intervene, as well as systemic failures in pilot screening and FAA oversight, as contributing factors.
